Pediatric Urology in Türkiye | MPGCARE
Pediatric urology assesses urinary tract and genital conditions from infancy through adolescence. The priorities are protecting kidney function, enabling safe bladder emptying and supporting the child’s development. Some conditions require observation or medicines; others need timely surgery. A finding on an ultrasound does not automatically mean an operation is necessary.
MPGCARE coordinates specialist review at Medical Park and Liv Hospital in Türkiye. The hospital confirms the appropriate doctor, procedure and availability for the child’s case. We support records, appointments, communication and travel. Complex conditions may involve pediatric nephrology, surgery, radiology or other teams.
Conditions assessed by pediatric urology
These are common and selected complex consultation topics. The individual doctor’s scope and available treatment are confirmed after document review. This guide is not a diagnosis or a promise that every intervention is offered at every hospital.
| Disease / condition | What it means | Evaluation and treatment approach |
|---|---|---|
| Hypospadias | The urinary opening is below its usual position; curvature or foreskin differences may coexist. | Examination guides whether repair is needed, its timing and whether one or staged procedures are appropriate. |
| Congenital penile curvature | Curvature can occur with or without hypospadias. | Assess anatomy and functional implications; correction is individual rather than automatic. |
| Undescended testis (cryptorchidism) | One or both testes are not in the scrotum. | Timely examination and orchidopexy planning when indicated; do not delay specialist review. |
| Retractile or ascending testis | A retractile testis differs from a persistently undescended or newly ascending one. | Clinical examination and follow-up; treatment depends on the confirmed position over time. |
| Hydrocele | Fluid around a testis causes scrotal swelling. | Differentiate communicating and non-communicating forms; observation or repair according to age and findings. |
| Inguinal hernia | A groin passage allows tissue to protrude. | Pediatric surgical assessment; painful trapped swelling or vomiting needs urgent care. |
| Testicular torsion | Twisting can interrupt blood supply to a testis. | An emergency: sudden scrotal pain requires immediate local hospital assessment. |
| Varicocele in adolescents | Enlarged veins around the testis. | Review symptoms and testicular development; monitoring or selected treatment. |
| Phimosis | Foreskin tightness may be normal development or pathological scarring. | Do not force retraction; observation, prescribed topical treatment or selected surgery. |
| Paraphimosis | Retracted foreskin becomes trapped behind the glans. | Urgent local treatment; do not wait for travel. |
| Buried penis and penile adhesions | Skin attachment or concealment may affect hygiene or function. | Individual examination; conservative care or reconstruction when justified. |
| Meatal or urethral narrowing | Narrowing can affect urinary stream and emptying. | Assess flow and anatomy; selected procedures if clinically significant. |
| Hydronephrosis | Dilatation of the kidney’s collecting system, sometimes detected before birth. | Follow ultrasound and function; distinguish temporary dilatation, obstruction and reflux. |
| UPJ obstruction | Drainage may be restricted where the kidney joins the ureter. | Serial assessment and functional imaging when needed; pyeloplasty for suitable significant obstruction. |
| Megaureter and UVJ obstruction | An enlarged ureter or restricted drainage near the bladder. | Monitor infection, drainage and kidney function; selected reconstructive treatment. |
| Vesicoureteral reflux (VUR) | Urine flows backwards from the bladder towards a kidney. | Grade, infections and kidney findings guide observation, prevention or selected injection/surgery. |
| Duplex kidney, ectopic ureter and ureterocele | Developmental drainage differences may cause infection, obstruction or continuous wetting. | Anatomy and renal function determine observation, endoscopic or reconstructive care. |
| Posterior urethral valves | Obstructive tissue in boys can affect bladder and kidneys. | Prompt evaluation and relief of obstruction; long-term bladder and kidney follow-up. |
| Recurrent urinary tract infection | Repeated infections, especially with fever, need confirmation and assessment. | Urine culture and selected imaging; review bladder emptying, constipation and structural causes. |
| Bedwetting (nocturnal enuresis) | Night wetting beyond the expected developmental stage. | Supportive assessment, routines and selected alarm or prescribed treatment; never blame the child. |
| Daytime wetting and dysfunctional voiding | Urgency, leakage or incomplete emptying may be linked to bladder-bowel dysfunction. | Diary, bowel management and urotherapy; selected tests or medicines when indicated. |
| Neurogenic bladder | Nerve-related bladder dysfunction may threaten emptying or kidney safety. | Individual pressure/function assessment, catheterisation training or other treatment; ongoing surveillance. |
| Kidney and urinary tract stones | Crystals form stones that can cause pain, blood or blockage. | Assess stone and metabolic causes; observation, removal or fragmentation plus recurrence prevention. |
| Bladder exstrophy and epispadias | Complex congenital bladder or urinary-opening abnormalities. | Specialist multidisciplinary reconstruction and long-term continence planning. |
| Urinary or genital trauma and suspected tumours | Injury, a persistent mass or blood in urine may need specialist evaluation. | Urgency depends on presentation; imaging and multidisciplinary diagnosis rather than assuming cancer. |
What happens at the first consultation?
The doctor reviews prenatal findings, infections, urinary stream, wetting, bowel habits, previous surgery and growth. Bring ultrasound images and reports, urine cultures, renal-function results and a medication list. A voiding and bowel diary can help. Examination is age-appropriate, with caregiver involvement and respect for the child’s privacy. The plan is explained in language the family can understand.
Which tests may be needed?
Urinalysis, culture and kidney/bladder ultrasound are selected according to the problem. Uroflowmetry and residual-urine measurement can assess emptying in suitable children. VCUG may be needed for selected reflux or urethral questions; renal scans assess function or drainage in certain cases. Urodynamics is reserved for relevant bladder problems. Not every child needs all these tests, radiation or sedation; the team explains the reason and preparation.
Hypospadias repair and circumcision
The surgeon evaluates opening position, curvature, skin and urine flow. Some mild cases may not require repair; other cases need one or more operations. If hypospadias or another penile abnormality is suspected, defer circumcision until specialist review because foreskin may be needed for reconstruction. Catheter care, fistula, narrowing, persistent curvature and possible revision are discussed. Neither timing nor outcome can be guaranteed from a photograph alone.
Why is undescended testis assessment timely?
The specialist distinguishes an undescended testis from a retractile one and considers age and position. When surgery is indicated, orchidopexy places the testis in the scrotum; an abdominal or non-palpable testis may require a different approach. Earlier appropriate management supports future monitoring and fertility potential, but does not eliminate every long-term risk or guarantee fertility. The treating team sets timing and follow-up.
Does hydronephrosis or reflux always need surgery?
No. Some findings improve with growth and can be monitored, while significant obstruction, recurrent febrile infection or concerning functional changes may require intervention. Ultrasound appearance alone does not decide the operation. For VUR, grade, kidney findings and bladder-bowel function matter. Antibiotic prevention, endoscopic injection or reimplantation are selected individually. Continue the local monitoring plan while seeking another opinion.
Bedwetting, daytime leakage and constipation
Wetting is not laziness or misbehaviour. The team differentiates isolated night wetting from daytime symptoms, infection or other disease. A supportive routine, appropriate fluid intake and constipation treatment may be part of care. Alarms or prescribed medicines are considered according to age and pattern. Sudden excessive thirst, increased urination, weight loss or new illness needs prompt medical review rather than being labelled ordinary bedwetting.
How are stones and neurogenic bladder managed?
Stone treatment considers size, location, obstruction, infection and possible metabolic causes. Selected endoscopic or other procedures may be needed; prevention should protect normal childhood nutrition rather than use unsupervised restrictive diets. Neurogenic bladder care focuses on safe pressures and emptying, sometimes with catheterisation, medicines or reconstruction. Parents receive practical training and a kidney-monitoring plan.
Surgery, anaesthesia and recovery
The team discusses why surgery is recommended, alternatives, expected benefit, risks and possible additional stages. Open, endoscopic or laparoscopic approaches are chosen for anatomy and expertise, not simply for a smaller incision. Pediatric anaesthesia and preoperative preparation are individual. Pain care, catheter or stent removal, bathing, activity and return to school are written in the discharge plan. Stays and travel clearance depend on the procedure and recovery.
Which symptoms require immediate local care?
Sudden testicular or scrotal pain, painful trapped groin swelling, inability to urinate, severe flank pain with fever or a very unwell infant require urgent assessment. Testicular torsion is time-critical even if the pain starts to improve. Seek a local emergency department; do not wait for a message, overseas appointment or flight. Postoperative fever, worsening pain or catheter problems should be reported according to the treating team’s instructions.
Will follow-up continue after treatment?
Yes, follow-up can include wounds, urine cultures when indicated, ultrasound, drainage, kidney function and bladder habits. Some congenital and neurogenic disorders require surveillance through adolescence and transition to adult care. An operation may correct anatomy without resolving every functional issue. Before leaving, agree which reviews are needed in Türkiye and which can be performed by local clinicians, and obtain the medical summary.
What documents and costs should families clarify?
Provide age, symptoms, diagnoses, prenatal and current imaging, cultures, previous operations and medicines through MPGCARE’s designated channel. The hospital reviews suitability and the appropriate specialist. Before booking, clarify consultation, tests, surgery, anaesthesia, hospital stay, catheter/stent removal and follow-up in an itemised estimate. A preliminary review supports planning and does not replace examination or promise surgery.
Travel and family coordination
MPGCARE offers free Sarp–Trabzon transfer for road arrivals and free airport–hospital transfer for air arrivals. Timing, route and accompanying-person arrangements are confirmed beforehand. Accommodation and appointments are planned around the child’s needs. Return travel follows the clinician’s approval; required postoperative reviews should be included in the itinerary.
