Thoracic Surgery
Thoracic surgery evaluates selected diseases of the lungs, pleura, mediastinum, chest wall, diaphragm and airways, with specialist pathways for some oesophageal conditions. An operation may obtain a diagnosis, remove disease, relieve a complication or support symptom control. Suitability depends on the condition, extent of disease, breathing reserve and overall health.
MPGCARE coordinates review at Medical Park and Liv Hospital in Türkiye. The receiving hospital confirms the surgeon, appropriate procedure and available techniques. We help organise medical records, communication, appointments and travel. Pulmonology, oncology, radiology, anaesthesia and other teams may contribute to a personalised plan.
Conditions assessed in thoracic surgery
These are consultation topics, not an indication that every case requires surgery. Some procedures are appropriate only in carefully selected patients or specialist centres. The hospital confirms scope and suitability after reviewing the case.
| Disease / condition | Clinical context | Possible surgical assessment |
|---|---|---|
| Non-small-cell lung cancer | Treatment depends on diagnosis, stage and general fitness. | Selected resection with lymph-node assessment; tumour-board review of surgery and other therapies. |
| Lung nodules and indeterminate masses | A nodule is not automatically malignant. | Risk-based surveillance, biopsy or selected removal. |
| Selected lung metastases | Cancer in the lung may originate elsewhere. | Metastasectomy considered only after oncology review and assessment of disease elsewhere. |
| Pneumothorax and lung bullae | Air leakage can collapse the lung. | Urgent care where needed; selected bullectomy or pleurodesis for appropriate indications. |
| Pleural effusion | Fluid may have infectious, malignant or other causes. | Sampling, drainage or selected procedures for recurring fluid after cause assessment. |
| Empyema | Infected fluid around the lung. | Antibiotics and drainage; selected debridement or decortication. |
| Pleural tumours and mesothelioma | Specialist tissue diagnosis and staging are important. | Multidisciplinary treatment; surgery is not a routine solution for every patient. |
| Mediastinal masses and cysts | Lesions between the lungs have different origins. | Imaging, selected biopsy or removal based on anatomy and diagnosis. |
| Thymoma and thymic carcinoma | Tumours of the thymus require staging. | Resectability and additional oncology treatment are reviewed individually. |
| Selected myasthenia gravis cases | Some patients may benefit from thymus surgery. | Neurology and thoracic surgery decide candidacy and perioperative preparation. |
| Chest-wall tumours | May arise from bone or soft tissue. | Tissue diagnosis, oncology review and selected resection/reconstruction. |
| Pectus excavatum and carinatum | Chest-wall shape and functional impact vary. | Assess severity, symptoms, age and alternatives before any correction. |
| Rib fractures and chest trauma | Injury may affect breathing or internal structures. | Emergency assessment; selected fixation or surgery according to injury. |
| Haemothorax | Blood collects around the lung. | Urgent local care; drainage and selected surgical control. |
| Diaphragm disorders | Weakness, elevation or a defect may affect breathing. | Cause and functional assessment; selected repair or plication. |
| Tracheal or bronchial narrowing | Airway narrowing has varied causes. | Multidisciplinary airway assessment and selected reconstruction or other treatment. |
| Oesophageal surgical conditions | Cancer or selected structural disease needs specialist review. | Coordinate gastroenterology, oncology and appropriate surgical expertise; availability confirmed. |
| Selected localised bronchiectasis or complicated cystic/infectious disease | Medical treatment is usually considered first. | Surgery only when localisation, symptoms and breathing reserve justify it. |
How is a surgical opinion prepared?
The surgeon reviews symptoms, previous operations, smoking, medicines, imaging and pathology. Bring CT images, reports, biopsy results and lung-function tests; cancer cases may also require staging information. The purpose of each proposed test is explained. Document review can guide the next appointment but cannot replace examination or promise an operation.
Diagnosis, staging and the multidisciplinary team
Imaging, bronchoscopy, EBUS, image-guided biopsy or other sampling is selected according to the clinical question. Not every patient needs PET-CT, MRI or every biopsy method. For suspected cancer, tissue type, lymph nodes and distant spread inform treatment. The team considers surgery, systemic therapy, radiation, observation or symptom-focused care; the order may change after full staging.
Can my lungs tolerate an operation?
Spirometry, diffusion capacity and selected exercise testing help assess respiratory reserve; heart health and anaesthetic risk also matter. The team considers the amount of lung proposed for removal, nutrition, mobility and other illnesses. Smoking cessation and appropriate preparation can support recovery. A small tumour does not by itself establish surgical fitness.
VATS, robotic surgery and thoracotomy
VATS uses a camera through smaller chest incisions; robotic assistance is another approach in selected procedures where available. Thoracotomy uses an open incision and can be the most appropriate option for complex anatomy or extensive disease. Smaller incisions do not remove surgical risks. Conversion to open surgery may be needed for safety. Technique is chosen for diagnosis, anatomy, expertise and availability.
How much lung might be removed?
Wedge resection removes a small non-anatomical piece; segmentectomy removes an anatomical segment; lobectomy removes a lobe. Pneumonectomy removes an entire lung, while a sleeve procedure may reconstruct an airway in selected cases. These operations have different implications and are not interchangeable. For cancer, margins and lymph-node assessment matter; breathing reserve and oncological suitability guide the choice.
Pleural and mediastinal procedures
Drainage can relieve air or fluid; recurrent fluid or air leakage may justify other measures such as pleurodesis in suitable patients. Empyema sometimes needs removal of infected material or a restrictive layer. Mediastinal and thymus surgery is based on anatomy and diagnosis. Thymectomy for myasthenia requires neurological planning and is not recommended for every form of the disease.
What are the risks and alternatives?
Discuss the expected benefit and alternatives for the exact procedure. Risks can include bleeding, infection, pneumonia, prolonged air leak, blood clots, rhythm disturbance, chronic pain, respiratory failure, reoperation and death. Individual risk depends on the operation and health, so a generic percentage is not a personalised prediction. The surgeon explains what happens if surgery is unsuitable or findings change during the operation.
Chest drains, pain relief and inpatient recovery
Chest drains may remove air, fluid or blood and are monitored until removal criteria are met; duration varies and some patients need additional support. Pain relief helps breathing, coughing and movement. Physiotherapy, breathing exercises and early safe mobilisation support recovery. The team decides ward or intensive-care needs. Never remove, disconnect or adjust a drain without the treating team.
Home recovery and follow-up
Before discharge, obtain instructions for wounds, medicines, activity, lifting, driving and scheduled reviews. Fatigue and discomfort may take time to improve, and return to work is individual. Cancer surgery requires review of final pathology and any additional treatment. A written summary supports coordination with home-country clinicians. No fixed recovery date or cancer outcome can be guaranteed.
When should I seek urgent local help?
Sudden severe breathlessness, chest trauma, significant bleeding or coughing blood, fainting, confusion or rapidly worsening chest pain require urgent local care. After surgery, new breathlessness, fever, worsening wound symptoms or drain concerns should be reported immediately according to the discharge plan. Do not wait for a flight, overseas consultation or online reply.
Travel, stay and the cost estimate
Agree travel only after the team assesses clinical stability and, after surgery, provides clearance. Residual air, recent procedures and oxygen needs may affect flights. The itemised estimate should clarify tests, surgery, anaesthesia, ward/ICU, pathology, drains, medicines and follow-up, including how unexpected additional care is handled. The hospital confirms available techniques; an online enquiry does not reserve a procedure.
MPGCARE coordination
Send age, diagnosis, imaging/pathology, lung tests, previous treatments and current medicines through the designated channel. We coordinate specialist review and the proposed pathway. Free Sarp–Trabzon transfer is offered for road arrivals and free airport–hospital transfer for air arrivals; timing, route and companions are confirmed beforehand. Accommodation and required postoperative reviews are included in travel planning according to individual needs.
