Rheumatology and Autoimmune Disease Care
Rheumatology evaluates inflammatory joint disease, systemic autoimmune conditions and selected bone or pain disorders. MPGCARE coordinates consultations and records with Medical Park and Liv Hospital in Türkiye. The receiving physician establishes the diagnosis and treatment plan; pediatric cases need a specifically confirmed pediatric rheumatology service.
Joint pain has many causes. These tables explain common pathways, not tests that everyone must undergo. The clinician chooses investigations from your symptoms, examination and previous results.
Joint, spine and crystal-related conditions
| Condition | Symptoms to discuss | Selected investigations and care approach |
|---|---|---|
| Rheumatoid arthritis | Persistent swollen/painful joints and prolonged morning stiffness, often affecting hands/feet. | Examination, selected ESR/CRP, rheumatoid factor/anti-CCP and ultrasound/X-rays. Early disease-modifying treatment is individualized to control inflammation and limit damage. |
| Psoriatic arthritis | Joint swelling, swollen digits, tendon pain and psoriasis or nail changes. | Skin/joint examination, selected blood tests and ultrasound/MRI/X-rays. Treatment targets the affected joints, spine, tendons and skin. |
| Axial spondyloarthritis / ankylosing spondylitis | Persistent inflammatory back pain, morning stiffness or painful tendon insertions. | Clinical assessment, selected sacroiliac X-rays/MRI and HLA-B27 testing. HLA-B27 alone is not diagnostic; exercise and prescribed medicines are individualized. |
| Gout | Sudden intensely painful swollen joints, sometimes recurrent attacks or tophi. | Selected joint-fluid crystal analysis, urate/renal tests and imaging. Infection must be considered; flare care and long-term urate management are separate decisions. |
| Polymyalgia rheumatica | New shoulder/hip-girdle pain and stiffness, mainly in older adults. | Examination and inflammatory markers with assessment for alternative causes. Steroid treatment and tapering require monitoring; new headache, jaw pain or vision changes needs urgent review. |
| Osteoarthritis | Activity-related joint pain, stiffness and reduced function. | Clinical assessment; X-rays where they help decisions. Exercise, weight/function support and selected pain treatment are used; advanced cases may need orthopedic review. |
Autoimmune, bone and chronic pain conditions
| Condition | Symptoms to discuss | Selected investigations and care approach |
|---|---|---|
| Systemic lupus erythematosus (SLE) | Joint/skin symptoms, mouth ulcers, fatigue or organ involvement. | Selected ANA/specific antibodies, blood counts, complement, kidney/urine tests and organ-directed investigations. Treatment depends on disease activity and involved organs. |
| Sjögren’s disease | Dry eyes/mouth, fatigue, joint symptoms or salivary swelling. | Selected antibody tests, tear/saliva assessment and biopsy when indicated. Eye, oral/dental and systemic treatment are tailored to findings. |
| Systemic sclerosis / scleroderma | Raynaud’s symptoms, skin tightening or digestive/respiratory problems. | Clinical/antibody review, selected nailfold examination and lung/heart/kidney monitoring. Organ involvement guides treatment and ongoing surveillance. |
| Inflammatory myopathies | Muscle weakness, sometimes rash or swallowing/breathing problems. | Muscle enzymes, selected antibodies, MRI/EMG and biopsy where indicated. Specialist treatment and rehabilitation depend on the subtype and organ effects. |
| Systemic vasculitis | Inflammation affecting blood vessels, with varying skin, nerve, kidney, lung or other symptoms. | Selected blood/urine tests, organ imaging and sometimes tissue biopsy. Subtype and severity guide immunomodulatory care; organ-threatening disease requires urgent assessment. |
| Behçet’s disease | Recurrent oral/genital ulcers, eye inflammation or skin/joint symptoms. | Clinical criteria and investigations to assess affected organs/exclude mimics; no single definitive blood test. Eye, vascular or neurological symptoms require prompt specialist coordination. |
| Osteoporosis and fragility-fracture risk | Low-trauma fractures or risk factors such as prolonged steroid treatment. | DXA bone-density testing and selected tests for secondary causes. Fracture-risk assessment, fall prevention and prescribed bone treatment guide care. |
| Fibromyalgia | Widespread pain, fatigue, poor sleep and cognitive symptoms. | Clinical assessment and targeted tests for alternative/coexisting causes. It is not diagnosed by a single blood test; activity, sleep and individualized symptom care are central. |
Which records help the consultation?
Prepare a symptom timeline, photos of intermittent swelling/rashes, morning-stiffness duration and functional difficulties. Include dated blood/urine tests, images/DICOM files, biopsy reports, previous diagnoses and a full medicine/allergy list. Mention infections, TB/hepatitis history, pregnancy plans and family history. Use the agreed secure channel.
Does a positive ANA or rheumatoid factor prove a diagnosis?
No. An antibody result must be interpreted with clinical findings; positive ANA can occur without an autoimmune disease. A negative result or normal inflammatory markers does not exclude every rheumatic condition. Avoid ordering broad repeated antibody panels independently. Ask which finding changes your diagnosis or management and why a test needs repeating.
What is the goal of treatment?
For inflammatory disease, treatment aims to control activity, protect joints/organs and preserve daily function. Symptom relief alone may not show whether inflammation is controlled. Ask about the diagnosis, treatment target, alternatives and expected time to reassessment. Remission may be possible for some patients, but response and permanent cure cannot be promised.
Important reminders about prescribed medicines
Take medicines exactly as directed and keep the agreed monitoring appointments. Confirm the schedule, missed-dose instructions and side-effect contact plan. Some rheumatology medicines have unusual schedules; do not assume every medicine is taken daily. Do not abruptly stop steroids or alter immunosuppressive treatment independently. Tell the team about non-prescription pain medicines, supplements and new prescriptions.
Infection screening, vaccines and biologic treatment
Before selected biologic or immune-modifying medicines, the team may require TB/hepatitis and other safety checks. Discuss vaccinations before starting treatment; live-vaccine advice depends on the medicine and immune status. Report fever, significant infection or exposure promptly and ask whether a scheduled dose needs review. Do not make treatment changes without the prescribing team’s instructions.
Pregnancy, breastfeeding and planned operations
Discuss conception and contraception before changing or starting medicines; disease activity and certain drugs can affect pregnancy planning. A rheumatologist and obstetric team may need to coordinate care. For surgery or dental procedures, obtain individualized medicine instructions and disclose steroid use. Do not stop necessary treatment solely because travel, pregnancy or an operation is planned.
Movement, daily routines and family support
An individualized exercise/physiotherapy plan can support mobility and strength. Balance activity with rest during flares, discuss smoking cessation and look after bone/cardiovascular health. Family members can support medicine tracking and appointments with consent. Pain/fatigue deserve attention even when inflammation is controlled; complementary products do not replace disease treatment.
When is urgent local assessment needed?
A suddenly very painful, hot swollen joint—especially with fever—may be infection and needs urgent assessment. New vision loss, painful red eyes, new severe headache with jaw pain, chest pain, severe breathlessness, new weakness or rapidly worsening illness also needs prompt local care. Do not wait for an overseas appointment or assume every new symptom is a routine flare.
Follow-up after returning home
Agree who orders and interprets blood/urine monitoring, renews prescriptions and assesses flares locally. Obtain a written plan and access to urgent advice. Follow-up may include kidney, lung, eye or bone monitoring depending on the condition and medicine. Treatment is often long term; one consultation does not replace ongoing care.
Appointments, costs and transfers
MPGCARE team helps organize records and confirm the specialist center and written proposal. Clarify consultation, selected tests, medicine/infusion costs, repeat monitoring and follow-up separately. Complimentary Sarp–Trabzon and airport–hospital transfers can be coordinated where appropriate; routine transfers do not replace emergency medical transport.
